what is the common etiquette for cycling?

I’m a cross country runner and i’ve started to get into cycling and swimming for cross training. there are a few situations with cycling I’m confused about.

for example what should I do on a 2 lane road when a car behind me needs to pass?

i’ll be riding in the morning (6 AM-ish) so which would be better: a few lights, or a few reflectors?

we also have a six lane road. not much traffic at 6:00 but if I need to turn left at an intersection, how far from the intersection is it appropriate to be in the left lane?

also, anyone have any suggestions for a stationary trainer (the thing you put on the rear tire to adapt a road bike into a stationary)

Stay as far to the right as you can when a car needs to pass and that driver should only do so when they can otherwise you are perfectly legal being where you are on the road. If it is practical for you to move off the paved roadway to allow them room then do so otherwise you have the right to be there too.
Get a red flashing taillight and a good flashing headlight or a steady higher powered light (Nite Rider Sol) for maximum visibility. Reflectors are great but I strip them always. This light for economy purposes: http://www.performancebike.com/bikes/Product_10052_10551_1030710_-1_20000__22000
In a left turn situation you are bound by the same laws as a motorized vehicle and you may use the entire lane just as a motorcycle would to make a left turn so move over when the lane permits you too.
stationary trainer : I use this one: http://www.performancebike.com/bikes/Product_10052_10551_1028771_-1_400006_400006_400006 and nearly anyone will be fine. If you use it or will use it extensively then get a fluid trainer not a magnetic one (louder and dont last as long).
